You need to use the spoiler tags like this:

With title:
[spoiler=Title Here]Enter text here...[/spoiler]

Without title:
[spoiler]Enter text here...[/spoiler]

EDIT
For your example above, write exactly this:

[spoiler=Is it working?]Am v confused[/spoiler]

or without the title:

[spoiler]Am v confused[/spoiler]
